Amy is the CEO/Founder of Care Resource Connection a 501 c3 nonprofit. Our mission bridges the gaps between our communities and the resources they need. Our Care Coordination Model is considered the best practice among Community Health Programs, creating resilient communities that advance health equity in access to healthcare and services. We work directly with fire, law enforcement, and public safety partners to connect the right resources at the right time to better serve patient needs and decrease the overutilization of community emergency services. Care Resource Connection has developed a Community Health Toolkit with a focus on Community Risk Reduction.
