PRESENTERS

Katie Westman, DNP, CNS (she/her)
Katie is an adult-gero clinical nurse specialist with a career spanning acute care, facility and ambulatory based care, in both direct care and leadership. She has worked with populations with chronic and acute disease processes, run disease specific programs, and has experience with developing, improving and disseminating quality and safety work. She is excited to work on the new frontiers of healthcare, creating new ways for patients to receive care.

Tia Radant, MS, NRP (she/her)
Tia Radant is the Director of HealthPartners Hospital@Home and Community Paramedicine programs in Minnesota. Tia joined HealthPartners in 2014, and since then, she has developed a thriving community paramedicine program to address the post-acute needs of our patient population. She has also achieved national recognition for her work in the Acute Care at Home space. Tia has worked as a paramedic for urban and suburban 9-1-1 EMS services in the Twin Cities metro area and has over 25 years of experience in EMS, leadership, and education. Tia is a Nationally Registered Paramedic and holds a Master of Science degree in Experiential Education from Minnesota State University Mankato.

Janelle Shearer, RN, BSN, MA, CPHQ (she/her)
Janelle is a Program Manager at Stratis Health and has extensive experience leading quality improvement initiatives related to hospital, home health care, patient safety, and palliative care. During her 20 years at Stratis Health, she has led multiple community-based initiatives, a statewide coordination of care initiative, and co-led multiple rural community-based palliative care projects. She has over 20 years of experience as a registered nurse (RN) in a rural health system and has experience in multiple clinical areas, including Med-Surg, ICU/CCU, OB, house supervisor, and home health/hospice. Ms. Shearer holds a Master of Arts in nursing from Bethel University and a Bachelor of Science in Nursing (BSN) from the University of Iowa.

Chrisanne Timpe, MD (she/her)
Chrisanne is a hospitalist and medical director of several home-based medicine programs, including HealthPartners Hospital@Home, Home Based Medicine, and In-Home Complex Care. Her professional goals are built on the recognition that transitions of health are challenging and a desire to better support patients as they navigate acute care both in and out of the hospital and attempt to age in place.

Emily Downing, MD (she/her)
Dr. Downing is a board-certified Internist and Geriatrician. She serves as the System Clinical Officer for population health, home care services, and health equity for Allina Health. Dr Downing graduated from the University of Minnesota Medical School, and completed residency in Internal Medicine and Geriatrics Fellowship at the University of Colorado School of Medicine.
She is passionate about population health, care of seniors, care transitions, end of life care, and delivering care in the home whenever possible. She has developed and implemented new models of care in skilled nursing facilities, assisted living facilities and in the home including home based primary care and home hospital care.
